MEDICAL HISTORY AND CONSENT FORM

Please print, complete and give to your daughter's coach at the first practice.
 It is very important that this form be completed and given to the coach in the event of an emergency requiring emergency medical services. Parents may not always be present for practices and games and it is vital to your daughter's safety that we have record of this information.
